Waminda is seeking a compassionate and experienced Elders Care Coordinator to provide culturally safe, strengths-based aged care coordination and support to Aboriginal and Torres Strait Islander women aged 50 years and over Aunties). In this rewarding role, you will walk alongside Elders to strengthen their health, wellbeing, independence, and connection to community by supporting them to navigate aged care systems, access culturally appropriate services, and exercise choice and control in their care journey. You will also provide leadership and support to the Elders Care Connector team, ensuring high-quality, accountable, and culturally responsive service delivery.
What You’ll Be
Working within a strengths‑based, trauma‑informed, culturally safe framework, you will:
- Support Aboriginal and Torres Strait Islander Aunties to access aged care services that promote independence, safety, and wellbeing
- Assist clients to navigate My Aged Care, including registrations, assessments, referral processes, and service connections
- Develop personalised care plans in partnership with Elders that reflect their individual goals, needs, and cultural preferences
- Advocate for culturally safe and appropriate aged care services that uphold dignity, self-determination, and choice
- Coordinate referrals and collaborate with health, aged care, and community service providers to ensure seamless support
- Provide leadership, guidance, and supervision to Elders Care Connectors
- Build strong partnerships across the aged care sector and represent Waminda in relevant networks and collaborations
- Maintain accurate case notes, reporting, and documentation in line with program and funding requirement
What We’re Looking For
- You will identify as an Aboriginal and/or Torres Strait Islander woman, bringing living experience, and cultural insight, and community-connected practice to your work
- You will hold a diploma or above qualification in Health and Social Science and/or Community Services, or at least 3 years’ experience in a similar role
- You will have demonstrated experience providing aged care, care coordination, case management, or community support services to Aboriginal and Torres Strait Islander Elders
- You will bring strong knowledge of aged care systems and pathways, including My Aged Care (or the willingness to undertake training)
- You will demonstrate excellent communication, relationship-building, and partnership skills when working with Elders, families, communities, and service providers
- You will hold a current NSW Driver’s Licence and be willing to travel across service locations
- You will have or be willing to obtain a Working With Children Check and National Police Check
