Working within a Consumer Directed Care (CDC) model, we support people with disability, older persons, and young people with trauma backgrounds through flexible, person-centred services tailored to individual goals and aspirations.
We are seeking compassionate, adaptable, and motivated Support Workers to join our growing team across part-time, and casual opportunities.
About the Role
As a CHOICES & Aged Care Support Worker, you will provide high-quality, person-centred support to individuals with intellectual, physical, psychosocial, or multiple disabilities, as well as people who are ageing. You will work with minimal supervision using Active Support, Recovery-Oriented Practice, and trauma-informed approaches to promote independence, wellbeing, skill development, and community inclusion.
Support may be delivered in:
- Individual homes
- Residential settings
- Short-term accommodation
- Community environments
- Day programs
Shift lengths may vary from 1–8 hours and may include weekdays, evenings, weekends, sleepovers, and public holidays.
Key Responsibilities
- Provide direct support with daily living activities including personal care, domestic assistance, transport, social support, and skill development.
- Deliver supports in line with individual support plans, goals, and therapeutic frameworks.
- Support young people with trauma backgrounds using therapeutic and trauma-informed approaches.
- Promote choice, control, dignity, and independence in line with Consumer Directed Care principles.
- Encourage meaningful participation and valued roles within the community.
- Maintain accurate documentation, shift notes, and incident reporting.
- Work collaboratively with families, carers, allied health professionals, and internal teams
- Participate in training, supervision, meetings, and continuous improvement initiatives.
- Follow relevant legislation, policies, WHS requirements, and service standards.
About You
You are a caring and resilient professional who is passionate about supporting people to achieve their goals. You thrive in a team environment while also being confident working independently.
To be successful, you will have:
- Experience supporting people with disability, mental health challenges, trauma backgrounds, or ageing-related needs.
- A strong understanding of person-centred and trauma-informed practice.
- Excellent communication and relationship-building skills.
- The ability to respond calmly and effectively to complex situations.
- Strong time management and documentation skills.
- Confidence using smart devices and basic computer systems.
- Flexibility to work across a variety of shifts and locations.
You will also require:
- Minimum Cert III in disability, aged care or relevant area, or relevant work experience in disability services and/or aged care
- Current First Aid and CPR Certificate
- Current Working with Vulnerable People (WWVP) Card
- Current driver licence and access to a reliable vehicle
- Comprehensive motor vehicle insurance (includes business use)
- Ability to use a smart device in line with BYOD requirements
- NDIS Worker Screening Clearance(or willingness to obtain)
