About the Role

This role is responsible for delivering high-quality casework and support services to young people transitioning from state care to independent living. Working within a strengths-based and trauma-informed framework, you will provide practical support, advocacy, and mentoring to help young people achieve positive outcomes.

In addition to managing a caseload, you will support the Coordinator by mentoring and coaching new staff and students, leading reflective practice sessions, contributing to reporting requirements, and assisting with service administration. You may also act as the Coordinator’s delegate during periods of leave and represent Uplyft at community and stakeholder events.

About You

To be successful in this role, you will have:

  • A qualification in Social Work, Youth Work, Human Services, or a related discipline, and/or equivalent experience in the sector.
  • Demonstrated experience working with young people, ideally including those with experience in Out of Home Care.
  • The ability to support young people to develop practical life skills and successfully transition to independent living.
  • Strong case management, case recording, report writing, and individual planning skills.
  • An understanding of the complex relationships between young people, their families, carers, and support networks.
  • Knowledge of the cultural considerations and challenges Aboriginal young people may face when transitioning from care.
  • Proven ability to work collaboratively within a team and contribute positively to a supportive workplace culture.
