The Care Coordinator plays a key role in our Internal
Coordination team, managing a portfolio of approximately 100 aged care clients
to deliver high-quality, timely and well-coordinated services under the Support
at Home program. The role coordinates care services, manages client
relationships and budgets, maintains accurate documentation, ensures
compliance, responds proactively to enquiries, and aligns services with
clients’ needs, preferences and funding to support older Australians to live
independently at home.
What You’ll Do

Coordinate services, onboard new clients and proactively respond to changing needs to ensure continuity of care.

Handle high-volume calls (50-80 calls/day) and enquiries in a structured, fast-paced support environment

Build trusted relationships with clients, families and providers

Monitor client budgets, funding and billing to maximise available supports and ensure compliance.

Leverage Trilogy Care’s digital tools, including Microsoft 365, Zoho CRM and Care Vicinity, to maintain accurate client records, coordinate services and deliver exceptional operational
outcomes and timely support.

Identify risks, escalate complex matters appropriately and support clinical coordination where required.

Collaborate with internal teams, clinicians and service providers to deliver positive client outcomes.

Contribute to a culture of continuous improvement while maintaining compliance with workplace health, safety and
regulatory requirements.
