As a Register Nurse you will assist our clients with disabilities to take more control of their lives and better manage challenges of day to day living.
PURPOSE
• To monitor clinical practises ensuring compliance with practice standards and policies and procedures
• To oversee support of complex needs clients ensuring staff are supported in line with their individual needs
• To complete intake and develop care plan for complex needs clients
• To perform clinical work as required to meet needs.
• Undertake medication audits, assessment and training to ensure best practice is maintained
KEY RESPONSIBILITIES
- Managing clinical caseload involving the assessment, planning, implementation and evaluation of the appropriate treatment/management program.
- Selecting, applying and evaluating the effectiveness of interventions and their ability to meet goals and objectives in line with organisational practice standards.
- Encouraging client participation in program evaluation to ensure that services are relevant to the individual’s needs and goals.
- Contributing to the development and provision of community-based home support as relevant to the clients’ individual needs
- Working closely with Operations Manager and the Service Delivery team ensuring their cohesive approach to effectively support clients with complex support needs Overseeing practise to ensure high level of service delivery is being provided to Total Support Services clients.
- Oversight the clinical inbox and incidents ensuring systems supports remain compliant
- Ensure Risk Assessments are undertaken for both clients and properties and strategies for minimising risk are implemented.
- Ensure all client documentation is developed, reviewed, and updated as per schedule including Service Agreements, Support Plans, Medication Charts
- Oversee intake to ensure all documentation is complete as per policies and procedures and service delivery commences within two weeks.
- Oversee training to ensure staff maintain competency in medication management and clinical care.
