Telecare operates a hybrid GP practice based in Stanhope, delivering care through a combination of face to-face aged care visits and telehealth services. As part of our outreach program, you will support four aged care facilities across Victoria, alongside virtual care delivery.
The Opportunity
We are seeking a dedicated and flexible General Practitioner to join our hybrid care team. In this role, you will split your time between aged care outreach visits across four Victorian facilities and telehealth consultations, with the flexibility to structure your week around half-day or full-day telehealth sessions.

Key responsibilities
- Conduct quarterly face-to-face visits to four aged care facilities across Victoria, delivering comprehensive and compassionate patient care.
- Provide weekly telehealth consultations (half-day or full-day options available) to patients across the region.
- Deliver patient-centred care that meets the clinical and holistic needs of aged care residents and community patients.
- Maintain accurate clinical documentation and collaborate with facility nursing staff and allied health teams.
- Contribute to care planning, medication reviews, and case conferencing as required.
Remuneration
We offer competitive, transparent
remuneration with a clear billing structure:
- Aged Care (face-to-face), $250 per hour, 90% to GP (Medicare)
- Telehealth, $220 per hour, 75% to GP
Telehealth sessions are available in half-day
or full-day formats, giving you genuine flexibility in structuring your week.

Requirements
Essential
- Current AHPRA registration as a General Practitioner with no conditions affecting practice.
- Fellowship of the Royal Australian College of General Practitioners (FRACGP) or equivalent.
- Strong communication skills and confidence conducting telehealth consultations.
- Willingness and ability to travel to aged care facilities across Victoria for quarterly visits.
- A commitment to patient-centred, high-quality care in both residential and virtual settings.
Desirable
- Experience working in an aged care setting, including familiarity with care planning and ACFI/AN-ACC funding frameworks.
- Experience with telehealth platforms and virtual consultation best practice.
- Existing relationships or familiarity with aged care facilities in regional Victoria
