Community Worker: Support Coordination (Disability, Aged Care & Community Nursing)

Summary

National is a multidisciplinary provider delivering disability, aged care and community nursing services across the ACT and surrounding regions. We support participants with a broad range of support needs, including individuals living with complex disabilities, chronic health conditions and high-intensity support requirements. Our integrated service model brings together Community Workers, Registered Nurses, Allied Health Professionals and disability support teams to deliver coordinated, person-centred services that promote independence, safety and quality of life.

Our participant cohort includes individuals requiring complex community nursing, respiratory care, Hospital in the Home (HITH), tracheostomy airway management, palliative care and multidisciplinary clinical coordination.

About the Role

We are seeking an experienced Community Worker to join our Support Coordination team.

Working closely with participants, families, community nursing teams, allied health professionals, hospitals, government agencies and disability providers, you will coordinate services that enable participants to safely access the supports they need to achieve their goals.

This is an exciting opportunity to work within an organisation recognised for supporting participants with some of the most complex health and disability support needs in the community.

Key Responsibilities

You will:

  • Assist participants to understand and implement government-funded support plans.
  • Coordinate disability, aged care, community nursing and health services.
  • Advocate for participant choice, rights and independence.
  • Liaise with NDIA, Support at Home providers, hospitals, allied health professionals and community organisations.
  • Coordinate multidisciplinary services for participants with complex health and disability support needs.
  • Monitor participant outcomes, service quality and safeguarding requirements.
  • Maintain participant records, budgets, service agreements and care documentation.
  • Support participants during hospital discharge, service transitions and plan reviews.
  • Identify risks and coordinate appropriate responses.

 

About You

To be successful you will have:

  • Certificate IV and/or Diploma Qualification in Community Services, Disability, Social Work, Nursing or a related discipline.
  • Qualification in Psychosocial Supports.
  • Minimum three years’ experience within Community Services, Disability, Aged Care or NDIS Support Coordination.
  • Strong knowledge of the National Disability Insurance Scheme and Aged Care frameworks.
  • Excellent communication and stakeholder engagement skills.
  • Demonstrated ability to coordinate multidisciplinary services.
  • High level organisational and problem-solving skills.
  • Current Driver Licence.
  • National Police Check.
  • NDIS Worker Screening Clearance.
  • NDIS Worker Training Modules.
