Description

The South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West and the Great Southern region of Western Australia. SWAMS delivers a wide range of primary health and community programs and has a strong growth strategy through partnership opportunities and future community development.

This position delivers a quality social work service to Aboriginal people in the Mandurah region that is culturally appropriate, secure and respectful. This is a full time position working a 7.6 hour day – Monday to Friday.

You will be part of a multidisciplinary team providing assessments and coordination of physiotherapy programs including their ongoing evaluation, to improve or maintain levels of physical function for SWAMS clients, particularly those engaged with our Community Services, such as NDIS and aged care programs.

This role will provide culturally secure case management for clients and their family members through the provision of advocacy, follow-up, referral to appropriate services and collaboration with other support agencies. This will include:

  • Develop appropriate treatment plans with clients and family to assist them to navigate services
  • Liaise with health providers, other government and non-government agencies and communities
  • Provide brief intervention counselling, support, advocacy and appropriate therapeutic intervention.
  • Maintain reliable documentation
  • Provide qualitative and quantitative reports
  • Participate in research activities within the services as required
  • Participate in community development and capacity building activities

Essential Criteria

  • Tertiary qualification in Social Work eligible for full membership of the Australian Association of Social Workers (AASW) or the Society of Professional Social Workers (SPSW)
  • Experience in providing culturally appropriate support services, including case management, advocacy and supports to Aboriginal people
  • Demonstrated understanding of privacy and confidentiality issues.
  • Demonstrated effective communication and interpersonal skills
  • Ability to function independently or as part of a multidisciplinary team
  • Ability to travel throughout the South West and Bindjareb (Mandurah) regions.

Desirable Criteria

  • Aboriginal or Torres Strait Islander descent.
  • Knowledge and understanding of relevant legislative requirements.
  • Understanding of the inter-generational impacts of Stolen Generation policies
