We are seeking an experienced and motivated Case Manager to join our Therapeutic Residential Care team. This role provides an exciting opportunity to work directly with children and young people with complex needs, supporting their care journey and helping them achieve positive outcomes.
Working closely with Child Safety, residential care teams, families, schools, therapists and community stakeholders, you will play a critical role in ensuring each young person receives coordinated, responsive and therapeutic support.
About the Role
As a Case Manager, you will be responsible for coordinating all aspects of a young person’s care, ensuring their needs, goals and case plan outcomes remain at the centre of service delivery.
You will provide leadership and support around case planning, stakeholder engagement and therapeutic practice while working collaboratively with residential teams to promote placement stability, wellbeing and positive life outcomes.
This position is based from our Underwood office, however, regular travel is required across our residential care programs located throughout South-East and South-West regions. The successful applicant will be comfortable working across multiple locations and maintaining strong relationships with young people, staff teams, Child Safety and other stakeholders.
Key Responsibilities
- Coordinate case management activities for children and young people residing in residential care.
- Develop, implement, monitor and review individual behaviour support and case plans.
- Maintain strong relationships with Child Safety Service Centres and key stakeholders.
- Facilitate care team meetings and case discussions.
- Advocate for the needs and best interests of children and young people.
- Provide regular practice guidance, debriefing and support to residential care staff.
- Assist staff to reflect on practice, identify learning opportunities and respond effectively to complex behaviours and critical incidents.
- Support residential teams to implement therapeutic strategies and interventions.
- Participate in a rotating on-call roster, providing after-hours operational and practice support to residential care programs.
- Ensure documentation, reporting and case records are maintained to a high standard.
- Contribute to quality improvement and practice excellence initiatives.
About You
You will bring:
- Experience undertaking case management within residential care, child protection, youth services or a related field.
- Strong understanding of trauma-informed and therapeutic care principles.
- Knowledge of the Child Protection Act, Standards of Care, and Out-of-Home Care requirements.
- Excellent communication, stakeholder engagement and report-writing skills.
- Ability to work effectively with children and young people who have experienced trauma and adversity.
- Strong organisational skills and ability to manage competing priorities.
- Relevant qualifications in Human Services, Social Work, Psychology, Community Services, Behavioural Science or related discipline.
- Valid driver’s licence and willingness to travel as required.
