We’re now recruiting a Co-Care Partner to join our Perth team and help take PlanCare’s people experience to the next level.

This role is full-time, Mon–Fri, 9 AM to 5 PM based in our Perth office.

About the Team

ONEteam is PlanCare’s innovative partnership model designed for independent Care Managers who want to retain their client relationships while PlanCare provides the governance, compliance, funding administration and clinical support required under the Support at Home program.

As a Co Care Partner with ONEteam, you will become the primary point of contact for a portfolio of independent Care Managers, supporting them to navigate the operational, funding and compliance requirements of the Support at Home program while ensuring exceptional outcomes for the clients they support. This role combines relationship management, operational support, problem solving and quality oversight, working closely with both our partners and internal teams to deliver a seamless experience.

Key Responsibilities

Partner Relationship Management

  • Develop and maintain trusted relationships with a portfolio of independent Care Managers.
  • Act as the primary point of contact for operational support and guidance.
  • Provide advice on Support at Home processes, funding pathways and service delivery.
  • Assist partners to navigate complex client situations and operational challenges.
  • Build collaborative relationships that promote consistency, quality and positive outcomes.

Clinical & Care Governance

  • Support partners with complex client scenarios and funding enquiries.
  • Provide guidance on care planning, Support at Home legislation and quality standards.
  • Identify and escalate clinical, safeguarding or compliance risks where required.
  • Promote best practice across all aspects of care management.
  • Support partners to maintain high-quality documentation and client records.

Operational Excellence

  • Assist partners to navigate PlanCare systems, processes and operational requirements.
  • Liaise with Finance, Service Delivery, Clinical, Risk and Quality teams to resolve enquiries.
  • Support invoice, budget and funding enquiries.
  • Monitor operational issues and proactively work with internal teams to achieve timely resolutions.
  • Ensure governance, documentation and compliance requirements are consistently met.

Client Support & Escalations

While your primary relationship is with our Co-Care Partners, you will also:

  • Assist with client onboarding and budgeting, defining the role of PlanCare and the Independent Care Manager to clients.
  • Support the resolution of escalated client enquiries and complaints.
  • Work collaboratively with partners to resolve service delivery issues.
  • Help ensure clients receive safe, high-quality, person-centred care.

About You

You’ll thrive in this role if you enjoy building relationships, solving problems and supporting others to succeed. You will have a strong understanding of community aged care and are confident navigating funding, legislation and operational processes while maintaining a customer-focused approach.

Essential

  • Experience within Aged Care, Community Services or Health.
  • Strong knowledge of the Support at Home Program, Home Care Packages or community aged care.
  • Experience managing relationships with external stakeholders, providers or business partners.
  • Excellent communication and interpersonal skills with the ability to build trusted relationships.
  • Strong understanding of aged care legislation, compliance and funding requirements.
  • Highly organised with excellent time management and attention to detail.
  • Strong problem-solving skills and the ability to manage competing priorities.
  • Confidence using client management systems, Microsoft Office and digital platforms.

Highly Desirable

  • Clinical qualification (Registered Nurse, Enrolled Nurse or Allied Health).
  • Experience with My Aged Care, PRODA and Services Australia.
  • Experience supporting complex clients and navigating aged care funding.
  • Understanding of budget management within community aged care.
  • Experience working across multidisciplinary teams.
  • Experience in quality, governance or compliance.
  • Fluent in Chinese
