Social Worker – Veteran Wellbeing
$75,000–$80,000 per annum + Superannuation
Full-Time (Part-Time Considered)
Pivot Health Group is looking for a Social Worker to join our growing multidisciplinary allied health team. This is a unique role and not a traditional clinical social work position. Instead, you’ll focus on supporting the psychosocial wellbeing of our veteran clients through relationship building, regular check-ins, community engagement and helping them stay connected to their health goals.

You’ll work across our clinics in Rockingham, Baldivis, Port Kennedy and Mandurah, alongside Exercise Physiologists, Physiotherapists, Dietitians, Podiatrists, and soon, GPs.
About the Role
The majority of this role is spent engaging with veterans rather than conducting formal counselling appointments.
Veteran Psychosocial Support
You’ll work closely with our veteran clients by:
- Providing ongoing psychosocial support and wellbeing check-ins
- Building strong relationships with clients across our clinics
- Helping clients stay engaged with their health and wellbeing goals
- Assisting with referrals and linking clients with appropriate services
- Supporting clients with DVA paperwork and other relevant documentation when required
- Working collaboratively with our allied health team to coordinate care
- Maintaining accurate client notes
Social Engagement
You’ll also organise and run social activities every four to six weeks to help reduce social isolation and encourage connection. These may include:
- Coffee catch-ups
- Walking groups
- Cinema outings
- Local community activities
- Veteran social events
We also work closely with the Goldsworthy Veterans & Families Centre in Rockingham. You’ll spend approximately half a day each week at the Centre providing psychosocial support to veterans and their families.
About You
We’re looking for someone who enjoys working with people and is passionate about making a difference.
You’ll have:
- Membership with the Australian Association of Social Workers (AASW)
- CPR/First Aid
- Police Clearance
- Excellent communication and relationship-building skills
- The ability to work independently and as part of a multidisciplinary team
- Strong organisational skills
- A current driver’s licence and reliable vehicle
