Who is Mpower
Mpower is a well-established, accredited not-for-profit community-based organisation servicing South West Victoria. Established in 1975, Mpower has grown from a small voluntary service to an organisation employing over 95 staff, delivering a range of clinical and support services to more than 1,200 families across the region. With the introduction of the NDIS and expansion of community services, Mpower continues to grow and evolve to meet the needs of our local communities.
The Role
Our Carer Support Program provides person-centred support to individuals caring for others in the community. Carers may support people who are frail aged, living with disability, mental illness, dementia, chronic illness or terminal conditions. As a Carer Support Practitioner, you will play a key role in supporting carers through information, referrals, coordination, and access to flexible respite options. You will work directly with carers in their homes and community settings, building strong relationships and supporting them to achieve meaningful outcomes.
This is a full-time position based in Warrnambool, with some client related travel across the region.
About You
We’re looking for someone who is empathetic, proactive, and confident working with a diverse range of people. You will bring:
- Relevant qualifications in Disability, Social Work, Health, Welfare, Community Services or similar
- Experience in case management, community services, or working with carers
- A sound understanding of the challenges faced by carers and those they support
- Strong communication and engagement skills
- Ability to work autonomously and manage competing priorities
- Understanding of the challenges faced by carers and those they support
- Experience working collaboratively with internal teams and external stakeholders
What You’ll Be Doing
- Supporting carers through assessment, planning, and coordination of services
- Providing information, advocacy, and referrals to relevant supports
- Coordinating respite and other service options
- Facilitating carer activities, groups, and engagement opportunities
- Maintaining accurate case notes, reporting, and data integrity
- Building strong relationships with carers, service providers, and community networks

Employment terms and conditions are in line with the Social, Community, Home Care and Disability Services Industry Award 2010, with the applicable classification Level 3.
