The Role:

Due to growth, we have an exciting opportunity for a dedicated and compassionate Case Coordinator to join our team. In this role, you’ll coordinate care and services for our Commonwealth Home Support Program (CHSP) clients.

 

Key Responsibilities:

  • Conduct tele and in-home assessments to develop tailored, person-centred support plans in partnership with clients.
  • Oversee the delivery of services that align with clients’ goals and aspirations, ensuring their evolving needs are met.
  • Ensure compliance with aged care standards
  • Liaise with clients’ families and support networks to achieve positive outcomes.
  • Identify and manage risks to consumer safety.

This role is perfect for someone with strong organisational skills, a relationship-driven approach, and a passion for enhancing the lives of older South Australians.

 

About You:

  • Excellent communication and interpersonal skills.
  • Strong time management and prioritisation abilities.
  • Demonstrated empathy and flexibility in resolving sensitive issues.
  • Proficiency in Microsoft Office and experience with Client Management Systems.
  • Knowledge of aged care standards, CHSP and SAH is highly desirable.
