Description

The Take a Break Case Manager is responsible for delivering culturally safe, short-term case management within a low acuity withdrawal and stabilisation program. Operating under the direction of the AOD Program Manager, the role focuses on engagement, assessment, brief intervention, and transition planning for participants during their short stay (approximately up to 2 weeks). The position supports participants to stabilise, strengthen motivation for recovery, and actively progress toward ongoing treatment pathways, with a strong emphasis on facilitating entry into longer-term residential rehabilitation programs, including Venndale where appropriate.

Key Responsibilities:

  • Conduct timely intake, screening, and culturally appropriate assessments for all referred participants.
  • Provide pre-admission and admission engagement to ensure participants are suitable for low acuity withdrawal and short-stay support.
  • Deliver short-term, structured case management focused on stabilisation, engagement, and readiness for ongoing treatment.
  • Develop and implement individualised transition and support plans that prioritise referral into ongoing treatment pathways.
  • Facilitate low-risk withdrawal support and participant stabilisation within a safe, supervised residential environment.
  • Actively motivate and support participants to enter further treatment, with a strong focus on referral to residential rehabilitation services (including Venndale where appropriate).
  • Coordinate and maintain formal referral pathways and partnerships with internal and external service providers (e.g. rehabilitation, health, SEWB, housing, legal).
  • Facilitate “stepped care” transitions, supporting movement between withdrawal, rehabilitation, and community-based programs.
  • Support participants to access holistic services addressing Social and Emotional Wellbeing, including cultural, physical, and community needs.
  • Participate in collaborative care planning and case conferencing with partner agencies and clinical services.
  • Maintain accurate and timely client documentation and daily data entry into ESPCS in accordance with reporting requirements.
  • Monitor and document client progress, outcomes, and treatment completion status, including referrals and transition outcomes.
  • Ensure culturally safe, trauma-informed, and person-centred service delivery consistent with Social and Emotional Wellbeing principles.
  • Collect and respond to client feedback, contributing to continuous quality improvement and service adjustments.
  • Report all incidents and critical events within required timeframes in line with NT Health guidelines.
  • Monitor and report on service capacity and access issues, including wait times and inability to accept referrals.
  • Contribute to performance reporting requirements, including client entry, participation, exit outcomes, and treatment continuation rates.

Remuneration package:

Employment Type: Fulltime

Salary Range: $82,043.37 per annum (pro-rata)

  • 1 RDO per Month.

Location: This position is based at Venndale rehabilitation centre- Katherine NT 0850, Australia. Hours of work: 

Fulltime hours: 37.5 hours per week.

Monday – Friday | 08:00am – 4:30pm. with a 1hour unpaid lunch break.

Experience & Skills:

  • Certificate IV in Alcohol & Other Drugs
  • At least 2 years working in the AOD or Community Services or Client Focussed sector.
  • Experience working in AOD rehabilitation settings.
  • Knowledge and understanding of case management principles and practices.
  • Understanding of the cultural needs of Aboriginal and Torres Strait Islander clients.
  • Strong interpersonal skills, with the ability to work empathetically with clients from diverse cultural backgrounds.
  • Demonstrated ability to organise, facilitate, and participate in a variety of client-centred activities.
  • Strong administrative skills, including use of various IT equipment, devices and software systems.
  • Strong attention to detail and the ability to maintain accurate records.
  • Ability to work both independently and as part of a team.

 

Mandatory before commencement:

  • Valid NT Drivers Licence
  • Valid NT Working with Children Clearance (Ochre Card)
  • National Police Check
