As an approved DVA and Support at Home provider, we are passionate about helping clients remain safe, independent, and well-supported in their own homes while receiving clinically appropriate and person-centred care. 

We are currently seeking an experienced and compassionate Clinical Nurse to join our Townsville community nursing team. 

This role is ideal for a nurse who enjoys autonomy, meaningful client relationships, and the opportunity to make a genuine difference in the community while remaining supported by an experienced clinical leadership team. 

 

About the Role 

As a Clinical Nurse – Community Care, you will provide in-home clinical nursing services to veteran and aged care clients across the Townsville region. 

You will work collaboratively with Clinical Leaders, schedulers, GPs, allied health professionals, clients, and families to deliver safe, high-quality care in the community setting. 

This role combines hands-on clinical nursing with care coordination, clinical support, and a strong focus on holistic, person-centred care. 

 

Key Responsibilities 

  • Deliver in-home clinical nursing care to community clients 
  • Conduct nursing assessments and contribute to care planning 
  • Provide wound care, catheter care, medication support, and chronic disease management 
  • Monitor and escalate changes in client condition as required 
  • Maintain accurate and compliant clinical documentation 
  • Support best-practice clinical care and infection control standards 
  • Build positive relationships with clients, families, and healthcare stakeholders 
  • Provide guidance and support to other team members where appropriate 
  • Contribute to quality improvement and continuous improvement initiatives 

 

What We’re Looking For 

  • Current AHPRA registration as a Registered Nurse 
  • Minimum 3 years nursing experience 
  • Community, aged care, or home care experience highly regarded 
  • Experience in wound care and catheter management preferred 
  • Strong clinical assessment and documentation skills 
  • Ability to work independently and manage a community caseload 
  • Excellent communication and interpersonal skills 
  • Current driver’s licence and reliable vehicle 
  • Current National Police Check or willingness to obtain 
  • Full Australian working rights
