This is an exciting position working directly with community who present with a range of multiple and complex needs. This position works in a culturally safe, holistic and client-centered approach and is part of the new vibrant multidisciplinary Social and Emotional Wellbeing Team.

GEGAC is an Aboriginal Community Controlled Organisation based in Bairnsdale Victoria. Consisting of about 200 staff, GEGAC is a Not-for-Profit organisation that delivers holistic services in the areas of Primary Health, Social Services, Elders & Disability and Early Childhood Education.

GEGAC advocates for the self-determination of the Aboriginal community by prioritising services that enhance social, emotional, and spiritual wellbeing, with culture at the heart of everything we do.

 

Position Purpose

 The Case Coordination Practitioner – SEWB provides culturally safe, trauma-informed, and person-centred support to Aboriginal and Torres Strait Islander people presenting with multiple and complex needs.

The role delivers intensive case coordination, including:

•comprehensive SEWB and psychosocial assessment

•development and implementation of client-led case plans

•therapeutic and brief interventions

•warm and supported referrals

•aftercare planning

•ongoing monitoring of progress, risk, and wellbeing

 

This role strengthens social and emotional wellbeing by supporting individuals and families experiencing complex challenges, including mental health concerns, AOD issues, grief and loss, family stressors, identity and connection needs, and the impacts of intergenerational trauma.

Practice is guided by Aboriginal ways of knowing, being and doing, and by national frameworks including the National Strategic Framework for Aboriginal and Torres Strait Islander Peoples’ Mental Health and SEWB, the Cultural Respect Framework, and the National Drug Strategy

 

Key Selection Criteria 

·Demonstrated in-depth understanding of Aboriginal social and emotional wellbeing, including the impacts of intergenerational trauma, cultural identity, community dynamics, systematic disadvantage and culturally grounded healing frameworks.

·Proven experience delivering intensive support to clients with multiple and complex needs using coordinated, culturally safe and trauma-informed practice approaches.

·High-level capability to undertake comprehensive SEWB and psychosocial assessments, identify risks, and develop client-led case plans that integrate cultural strengths. Family involvement and appropriate supports.

·Demonstrated ability to deliver brief therapeutic interventions, including motivational interviewing and other evidence informed approaches, and to provide warm, supported referrals to culturally safe and mainstream services.

·Exceptional ability to build trust and therapeutic rapport with Aboriginal people, families and community members, particularly those experiencing trauma, grief, AOD concerns or mental health challenges.

·Strong professional integrity, including the ability to maintain confidentiality, uphold ethical boundaries, and apply sound judgement in complex and sensitive situations.

·High standard of documentation and case recording, including accurate, timely and culturally respectful case notes, assessments, plans and referral records.

·Competence and confidence using electronic client management systems and Microsoft Office applications, including the ability to meet reporting and data quality requirements.

·Understanding of governance, quality and safety frameworks relevant to primary health and community services (including Child Safe Standards, Social Services Standards, Victorian information sharing schemes, and clinical governance principles), and the ability to apply these in day-to-day practice.

 

Qualifications and Registrations Requirement (Essential or Desirable).

·Valid Victorian and unrestricted driver’s licence    ESSENTIAL

·Demonstrated understanding of Aboriginal culture, and the health and wellbeing needs of Aboriginal people    ESSENTIAL

·Certificate IV qualification (or equivalent experience) in Alcohol and Other Drugs, Counselling, Community Services/Social Work Support, Mental Health or Youth Work, demonstrating capability in trauma-informed practice, culturally safe client engagement, complex needs assessment, brief therapeutic intervention, coordinated care, risk identification, and the application of evidence-informed SEWB frameworks within primary and community health environments    ESSENTIAL

·A satisfactory Working with Children Check and Fit2Work Police Check    ESSENTIAL

·A diploma-level qualification (or equivalent experience) in Alcohol and Other Drugs, Mental Health, Counselling, Community Services, or a related behavioural or social science field, demonstrating advanced capability in trauma-informed practice, therapeutic engagement, complex case coordination, culturally safe service delivery, and the application of evidence-informed approaches within primary and community health settings    HIGHLY DESIRABLE

·A person of Aboriginal and or Torres Strait Islander background    HIGHLYDESIRABLE

·Membership (or eligibility for membership) with a relevant professional association such as ACWA, PACFA, ACA, NAATSIHWP, AASW, ANZMHA, or a recognised AOD or mental health professional network    HIGHLY DESIRABLE

·Proof of COVID-19 vaccinated to align with GEGAC requirements    ESSENTIAL
