Description

We are seeking a Graduate Registered Nurse to join the team at Focus Care. Reporting to the Clinical Quality Manager, you’ll gain valuable experience in community home care providing clinical care in disability and aged care settings while receiving ongoing training and mentoring.

What You’ll Do

  • Conduct clinical assessments in clients’ homes (initially under supervision)
  • Support care planning and clinical monitoring
  • Work alongside clients, families, GPs, allied health professionals and support workers
  • Assist with client education and self-management strategies
  • Participate in quality improvement and clinical review activities
  • Develop your skills caring for clients with diverse and complex needs

About You

  • Current AHPRA registration as a Registered Nurse
  • Recent graduate or early-career nurse
  • Passion for person-centred care in the community
  • Excellent communications skills (verbal and written)
  • Current Driver’s Licence
  • NDIS Worker Screening Check (or willing to obtain)
