Summary:

The Services Delivery Manager – Disability Home Care plays a crucial role in overseeing the delivery of high-quality services within the home care, disability sector.

Based in Indooroopilly, this permanent full-time position is responsible for supporting safe, consistent and person-centred service delivery across our disability support teams. The successful candidate will lead and support staff, oversee day-to-day service operations, monitor care quality, and work closely with participants, families, clinical staff and other stakeholders to ensure services are delivered to a high standard.

This role will suit an experienced disability, home care or community services professional who is ready to take greater responsibility for service delivery, team leadership and operational quality.

About You:

You are an experienced disability support professional who is ready to lead others and take greater responsibility for service delivery.

You will bring:

  • demonstrated experience in disability support, home care, SIL or community-based support
  • confidence and proven ability to lead, direct or supervise support workers
  • sound understanding of care plans, participant safety and professional boundaries
  • practical rostering, administration and documentation skills
  • experience using relevant client management, rostering or records management software
  • ability to communicate professionally with staff, participants, families and external providers
  • good judgement and the ability to make decisions within established procedures
  • a calm, mature and accountable leadership style
  • willingness to provide direct support when the service requires it

You will understand that quality care requires more than good intent. It requires consistency, documentation, follow-through, professional boundaries and the ability to support staff while holding them accountable to expected standards.

Responsibilities:

In this role, you will:

  • oversee the delivery of disability support services, ensuring compliance with relevant standards, policies and procedures
  • lead, guide and support disability support workers to deliver safe, respectful and high-quality care
  • monitor service quality, participant outcomes, documentation, incidents, risks and safeguarding concerns
  • oversee the implementation of participant care plans and support requirements
  • manage rostering, staffing coverage and day-to-day service coordination
  • support staff training, development, performance and supervision
  • work closely with participants, families, guardians, support coordinators, plan managers and clinical staff
  • identify areas for service improvement and support practical solutions
  • act as a point of contact for participant, family and staff feedback or concerns
  • assist with budget awareness, resource use, consumables and operational requirements
  • maintain accurate records, reports and service documentation
  • support compliance with NDIS Practice Standards, safeguarding requirements and relevant reporting obligations
  • provide direct support to participants when required to maintain continuity and quality of care.

Qualifications:

The successful candidate will ideally have:

  • Certificate IV in Disability Support, Individual Support, Community Services or a related field
  • a minimum of 5 years’ experience in disability, home care, community care or a related service delivery environment
  • previous experience in a senior support worker, team leader, service coordinator or service delivery management role
  • A qualification in health services, nursing, community services, disability, mental health or a related field will be highly regarded.
