About the Role 

Chosen Family is seeking empathetic and culturally responsive Registered Nurses (RNs) committed to delivering inclusive nursing supports to LGBTQIA+ participants across disability and community settings. 

The RN will provide clinically safe, trauma-informed, and affirming care while supporting participants who may have experienced discrimination, exclusion, complex trauma, psychosocial disability, or barriers accessing healthcare services. 

Key Responsibilities 

Clinical Nursing Supports 

  • Conduct nursing assessments and participant health reviews  
  • Deliver complex clinical supports including:  
  • Medication administration  
  • PEG feeding  
  • Wound care  
  • Catheter management  
  • Diabetes management  
  • Seizure management  
  • Mental health monitoring  
  • Continence care  
  • Behavioural observation and support  
  • Monitor participant wellbeing and clinical presentations  
  • Escalate concerns and coordinate appropriate interventions  
  • Deliver trauma-informed and identity-affirming healthcare supports  

LGBTQIA+ Inclusive Participant Support 

  • Foster emotionally safe and inclusive support environments  
  • Respect participant identities, pronouns, and lived experiences  
  • Support participants experiencing:  
  • Psychosocial disability  
  • Trauma  
  • Social isolation  
  • Mental health challenges  
  • Discrimination-related stress  
  • Promote participant autonomy, dignity, and self-determination  
  • Advocate for equitable and accessible healthcare services  

Care Planning & Clinical Oversight 

  • Develop and review participant care plans and support strategies  
  • Provide guidance and clinical oversight to support workers and ENs  
  • Participate in multidisciplinary care coordination  
  • Contribute to safeguarding and risk management processes  
  • Promote culturally safe and inclusive support delivery  

Stakeholder Engagement 

  • Liaise with families, guardians, allied health professionals, and community supports  
  • Collaborate with LGBTQIA+ inclusive services and healthcare providers  
  • Maintain professional and compassionate communication  
  • Participate in participant case conferences and support reviews  

Compliance & Documentation 

  • Maintain accurate clinical documentation and participant records  
  • Ensure compliance with NDIS Practice Standards and clinical governance frameworks  
  • Participate in audits, reviews, and quality improvement activities  
  • Maintain confidentiality and culturally safe practices  
  • Support continuous improvement in inclusive service delivery  

Essential Criteria 

  • Bachelor of Nursing  
  • Current AHPRA Registration as a Registered Nurse  
  • Current NDIS Worker Screening Check  
  • Current First Aid & CPR  
  • Strong understanding of trauma-informed and inclusive practice  

Highly Regarded 

  • Experience supporting LGBTQIA+ communities  
  • Mental health nursing experience  
  • Experience supporting transgender and gender-diverse participants  
  • Psychosocial disability or behavioural support experience
