About the Role

Chosen Family is seeking compassionate and participant-focused Enrolled Nurses (ENs) to provide high-quality nursing and disability supports across home, community, respite, and Supported Independent Living (SIL) environments. 

The EN will work collaboratively with participants, families, Registered Nurses, allied health professionals, and support teams to deliver safe, person-centred, and trauma-informed care that promotes participant wellbeing, independence, dignity, and quality of life. 

Key Responsibilities

Clinical Nursing Support

  • Deliver safe and effective nursing supports within EN scope of practice  
  • Assist participants with:  
  • Medication administration and monitoring  
  • Personal care and hygiene supports  
  • Mobility and manual handling supports  
  • Continence management  
  • Diabetes monitoring  
  • Vital observations  
  • Nutrition and hydration support  
  • Support delegated clinical procedures including:  
  • Wound care  
  • Catheter care  
  • PEG feeding assistance  
  • Bowel care  
  • Pressure area care  
  • Stoma care support  
  • Monitor participant health and wellbeing  
  • Identify signs of deterioration, behavioural escalation, or health risks  
  • Escalate clinical concerns appropriately to Registered Nurses or emergency services  
  • Support infection prevention and control procedures  

Participant-Centred Support

  • Promote participant dignity, choice, and autonomy  
  • Support participants to work towards individual goals and care outcomes  
  • Encourage independence and capacity-building  
  • Support community access, appointments, and social participation  
  • Build positive and therapeutic relationships with participants  
  • Maintain respectful, culturally safe, and trauma-informed support practices  
  • Support participant emotional wellbeing and confidence-building  

Care Planning & Service Coordination

  • Follow participant care plans, behaviour support plans, and risk assessments  
  • Contribute to participant reviews and care discussions  
  • Work collaboratively with Registered Nurses and multidisciplinary teams  
  • Support continuity of care across shifts and service settings  
  • Escalate incidents, safeguarding concerns, and hazards appropriately  
  • Assist with implementation of positive behaviour support strategies where required  

Stakeholder Engagement 

  • Liaise professionally with participants, families, guardians, and carers  
  • Communicate effectively with Support Coordinators and allied health professionals  
  • Participate in handovers, case discussions, and team meetings  
  • Maintain professional, compassionate, and responsive communication  
  • Support positive participant and family experiences  

Compliance & Documentation

  • Maintain accurate progress notes and clinical documentation  
  • Complete medication records, incident reports, and shift notes  
  • Ensure supports align with NDIS Practice Standards and organisational policies  
  • Maintain participant confidentiality and privacy  
  • Participate in audits, compliance activities, and ongoing training  
  • Ensure safe work practices and adherence to infection control standards  

Essential Criteria

 Diploma of Nursing  

  • Current AHPRA Registration as an Enrolled Nurse  
  • Current NDIS Worker Screening Check  
  • Current Working With Children Check  
  • Current First Aid & CPR Certificate  
  • Valid Driver’s Licence and reliable vehicle  
  • Strong communication and interpersonal skills  

 Desirable Criteria

  • Experience in disability or community nursing  
  • Experience supporting participants with high-intensity supports  
  • SIL or complex care experience  
  • Mental health or psychosocial disability experience  
  • Understanding of trauma-informed and person-centred practice
