CatholicCare Central Queensland is seeking an experienced and compassionate Dementia Together Consultant to provide personalised, evidence-informed advice and support to people living with dementia, their families and carers.
About the Role
The Dementia Consultant is responsible for providing personalised advice and guidance for people living with dementia based on contemporary evidence. This will be achieved by conducting assessments, interpreting health and clinical information, providing expertise to staff and clients and producing written reports that support the person living with dementia and their carers with practical recommendations and advice.
This role operates in accordance with the Dementia Together Service Delivery Model and provides non-diagnostic, evidence-informed advice and support within scope of practice and does not replace clinical or medical assessment.
Key responsibilities include:
🔹Coordinating, conducting, recording and interpreting a range of cognitive assessments and screening tools, demonstrating proficiency across face-to-face, phone and community-based settings.
🔹Empowering people living with dementia through personalised advice, guidance and reassurance;
🔹Supporting clients to understand their diagnosis, exercise choice, maintain meaningful relationships, and live well at home and in the community;
🔹Providing informed advice and guidance to carers based on contemporary evidence, equipping them with knowledge, strategies and confidence to sustain their caring role and respond effectively to changing needs;
🔹Collating high-quality, evidence-informed written reports and care recommendations that are clear, practical and accessible to clients, carers and service providers; and
🔹Contributing to team capability through sharing knowledge and supporting consistent practice approaches.
What We’re Looking For
Essential:
🔹Tertiary qualification in nursing, occupational therapy, social work or a related health or community services discipline, with current AHPRA registration or professional membership where applicable;
🔹Formal training or experience in dementia-specific assessment tools (e.g MMSE, ACE-III, RUDAS, KICA); and
🔹Current Queensland C Class Driver Licence.
Desirable:
🔹 Post-graduate qualification in dementia care, gerontology, aged care, or a related field (e.g Master of Dementia Care or equivalent) (Highly desirable).
You will also demonstrate:
🔹Exceptional communication skills with the ability to explain complex clinical information in a clear and compassionate way.
🔹Experience supporting people living with dementia and their families using a person-centred, strengths-based approach.
🔹Strong clinical assessment, care planning and problem-solving skills.
🔹Experience providing behaviour support education and coaching for carers.
🔹Knowledge of My Aged Care and community service systems.
🔹The ability to independently manage a diverse caseload while maintaining high-quality documentation.
🔹A commitment to quality improvement and collaborative teamwork.
