Baldwin Living is looking for an experienced Registered Nurse to step into a pivotal Clinical Case Manager role within our HomeServe program. This is not a desk-only case management role.
You’ll be the clinical backbone of our home-care services – overseeing complex care, strengthening governance, mentoring teams, and ensuring our participants receive safe, high-quality, person-centred support in their homes.

You will be responsible for:
- Leading comprehensive clinical assessments and risk management
- Designing and overseeing complex, personalised care plans
- Providing clinical governance oversight across home-care services
- Managing incidents, investigations, and quality improvement initiatives
- Supporting compliance with the Aged Care Quality Standards (Clinical Care – Standard 5)
- Mentoring and guiding care staff and non-clinical Case Managers
- Acting as the escalation point for complex clinical matters
This role requires regular interstate travel to support services across locations. You must be comfortable travelling and working independently.
We’re looking for:
- Current AHPRA registration as a Registered Nurse (no restrictions)
- Strong experience in aged care, community care, or complex case management
- Demonstrated capability in clinical governance and risk management
- Confidence mentoring and influencing multidisciplinary teams
- Strong documentation and compliance capability
- High level of autonomy and accountability
- Current unrestricted driver licence (essential)
- Willingness and capacity to travel interstate regularly
