About At Home Together
At Home Together provides in-home and community-based support to people living with disability, older people, and clients requiring assistance to remain safe, independent and connected in their community.
We are seeking caring, reliable and professional Support Workers to join our team across the ACT, Queanbeyan and surrounding areas.
About the Role
As a Support Worker, you will provide person-centered support to clients in their homes and in the community. The role may include domestic assistance, personal care, community access, transport, meal preparation, social support and assistance with daily living tasks.
You will be expected to follow each client’s care plan, respect their choices and privacy, complete accurate progress notes, and report any concerns or changes in client wellbeing to the appropriate manager or coordinator. These duties align with your current position description, which includes supporting clients with community access, direct care, documentation, privacy, client safety and person-centred support.
Key Responsibilities

Provide respectful, person-centered support in line with client care plans.

Assist clients with daily living tasks, domestic assistance, personal care, transport and community access.

Support clients to maintain independence, dignity, choice and social connection.

Complete accurate progress notes and required documentation after each service.

Report any risks, incidents, changes in client wellbeing or concerns to the manager or coordinator.

Follow workplace health and safety, infection control, privacy and confidentiality requirements.

Work professionally with clients, families, carers, coordinators and other team members.

Attend required training, meetings and supervision where required.
About You
We are looking for someone who is caring, reliable, respectful and able to work independently in the community. You will need good communication skills, strong time management, attention to detail and a genuine interest in supporting people to live safely and independently at home.
Previous experience in disability support, aged care, community care or similar work is preferred, but not essential for the right applicant. Training, buddy shifts and support will be provided.
Essential Requirements

Right to work in Australia.

Current Australian driver’s license.

Access to a reliable registered and insured vehicle.

Current National Police Check, or willingness to obtain.

ACT WWVP card with NDIS activities marked “YES”, or willingness to apply.

Current First Aid and CPR, or willingness to obtain.

Ability to use a mobile app to view shifts, clock in and out, and complete progress notes.

Willingness to complete mandatory onboarding and training requirements.
Desirable Requirements

Certificate III in Individual Support, Disability, Aged Care or similar.

Experience working with people with disability, older people or people requiring in-home support.

Manual handling training.

Medication assistance training.

Infection control training.

Experience with personal care, mealtime support, transport or community access.
