Position Summary
We are seeking a passionate and experienced Registered Nurse for 1.0 FTE to deliver exceptional client-centered care and support. Reporting to the Care Services Manager and House Manager, this role is pivotal in building strong relationships with clients and their families to achieve the best possible health and wellbeing outcomes. Great position in Community based Nursing north of the river, based in Shenton Park.
About The Role
- Conduct comprehensive client assessments, considering physical, psychological, social, and cultural needs.
- Develop and evaluate individualised Care Plans in partnership with clients, families, carers, and health professionals.
- Perform specialty-specific procedures and monitor treatment responses, adjusting care plans as needed.
- Communicate effectively with clients and families, providing education and support.
- Ensure accurate and timely documentation of all care activities.
- Advocate for ethical decision-making and coordinate interdisciplinary care plans.
- Contribute to policy development, education initiatives, and research projects that reinforce Alzheimer’s WA’s leadership in dementia care.
About You
- Sound knowledge of Aged Care Quality Standards (ACQSC)
- Knowledge of the Support at Home Program (formerly HCP) and CHSP
- Demonstrated experience supporting people living with dementia
- Strong community-based clinical experience
- Advanced communication and stakeholder engagement skills
- Well-developed assessment and care planning skills
- Ability to work autonomously and collaboratively as part of a team
