We are currently seeking a qualified and reliable Physiotherapist to provide one home visit per month to a client in Beaconsfield, Victoria.

About the Role

This role involves providing monthly physiotherapy support in the client’s home. The physiotherapist will assess the client’s mobility, strength, balance, pain, safety, and functional needs, and provide appropriate therapy, exercise guidance, and recommendations to support independence at home.

The role is suitable for a physiotherapist who has experience working with older people, home care clients, mobility concerns, chronic pain, falls risk, or functional decline.

Key Responsibilities

Your responsibilities may include:

  • Conducting monthly physiotherapy home visits
  • Assessing mobility, strength, balance, pain and functional capacity
  • Developing and reviewing a simple home exercise program
  • Supporting the client to maintain or improve mobility and independence
  • Providing falls prevention strategies and education
  • Monitoring progress and updating recommendations as required
  • Liaising with the care team regarding client needs, risks, and changes
  • Providing brief progress notes after each visit
  • Making recommendations for equipment or further allied health input where required
  • Maintaining professional documentation in line with clinical and organisational requirements

Requirements

The successful applicant must have:

  • Current AHPRA registration as a Physiotherapist
  • Previous experience in aged care, community care, home care, disability, or rehabilitation
  • Ability to work independently in a client’s home
  • Strong communication and documentation skills
  • Current National Police Check or willingness to obtain
  • Current driver’s licence and access to a reliable vehicle
  • Right to work in Australia

Preferred Experience

Experience in any of the following areas will be highly regarded:

  • Aged care or Home Care Package clients
  • Chronic pain management
  • Osteoarthritis support
  • Falls prevention
  • Mobility and balance programs
  • Functional assessments
  • Home-based rehabilitation
  • Working with older adults in the community
