Aged Care Support Worker – Domestic Assistance & Community Access

About the role
Allegiance Heart & Home Care is seeking a caring, reliable and experienced Support Worker to provide ongoing in-home and community-based support to an older female client living near Hurtle Square in Adelaide CBD.
This role is suitable for someone who enjoys helping older people maintain their independence, remain connected with the community and feel supported in their own home.
The successful applicant will work with the same client each week, providing consistency and building a respectful and trusting working relationship.
Wednesday – Domestic assistance
The Wednesday shift will focus on supporting the client to maintain a clean, safe and comfortable home.
Duties may include:

Washing, drying, folding and putting away laundry

Changing bed linen when required
-
Washing dishes and cleaning the kitchen
-
Wiping benches, cupboards and household surfaces
-
Cleaning the bathroom and toilet
-
Vacuuming and mopping floors
-
Dusting and general household cleaning
-
Emptying household bins
-
Light tidying and organisation
-
Assisting with other reasonable domestic tasks identified in the client’s care plan
The worker must be able to complete household duties safely and to a high standard while respecting the client’s home, belongings and preferences.
Thursday – Social support and community access
The Thursday shift will focus on companionship, emotional support and helping the client remain active and socially connected.
Duties may include:

Providing friendly companionship and meaningful conversation

Supporting the client to attend local shops, appointments or community activities

Accompanying the client on short outings
-
Assisting with grocery shopping and carrying light items
-
Supporting the client to attend a café, park, library, pool or other preferred activity
-
Encouraging participation in hobbies and interests
-
Providing reassurance and emotional support
-
Assisting the client to safely use mobility aids during outings
-
Monitoring the client’s wellbeing and reporting any concerns to the Care Partner
The client has reduced mobility and may use a walker. The worker must be patient, attentive and confident supporting an older person who may experience pain, fatigue and reduced balance.
About the client
The client is an older woman who lives independently in an apartment in Adelaide CBD.
She values her independence and would benefit from assistance with household tasks, companionship and accessing the local community. She may also require support to attend activities that are important to her, including shopping and gentle community outings.
The worker will be expected to follow the client’s support plan, respect her choices and encourage her to complete tasks independently wherever possible.
Essential requirements
Applicants must have:

Previous experience in aged care, disability support, community care or home care

Experience providing domestic assistance in a client’s home
-
Experience with social support and community access
-
A current National Police Check
-
A current First Aid and CPR certificate
-
A valid Australian driver’s licence
-
Access to a reliable, registered and insured vehicle
-
Comprehensive vehicle insurance
-
The right to work in Australia

A reliable mobile phone
-
Good spoken and written English
-
The ability to complete progress notes after each shift

Availability for both the Wednesday and Thursday shifts
Desirable qualifications and experience
The following would be highly regarded:

Certificate III in Individual Support, Aged Care, Disability or Community Services
-
Experience supporting clients with reduced mobility
-
Experience assisting clients who use walkers or other mobility aids
-
Manual handling training
-
Experience supporting clients experiencing chronic pain or fatigue
-
Knowledge of falls prevention and safe community access
-
Experience supporting older people living alone

Key responsibilities
The Support Worker will be required to:

Deliver services in accordance with the client’s care plan

Support the client’s independence and personal goals

Maintain professional boundaries

Follow infection-control and workplace safety procedures

Complete accurate progress notes after every shift

Promptly report incidents, falls, hazards or changes in the client’s condition

Communicate professionally with the client, family and Care Partner
-
Maintain confidentiality at all times

Attend induction and relevant training as required
