What are we looking for?

We are seeking a compassionate, clinically skilled and proactive Registered Nurse who is passionate about delivering person-centred care in community settings. 

You will be confident managing a diverse caseload across our aged care and NDIS clients and have experience supporting individuals with complex clinical needs. You enjoy hands on treatment and evidence-based nursing care, while also providing education and mentoring to care teams, and contributing to quality improvement initiatives that enhance client outcomes . 

This is a full-time role and requires flexibility from the candidate to provide support to clients across the Illawarra and Shoalhaven. This role is ideal for a nurse who enjoys variety, autonomy and working closely with clients, families and multidisciplinary teams to achieve outstanding outcomes. 

The role involves: 

  • Conducting clinical assessments and developing person-centred care plans. 
  • Delivering evidence-based nursing care and managing complex clinical needs across aged care and disability services. 
  • Providing specialist clinical support including medication management, wound care, continence management, catheter care, PEG feeding and chronic disease management. 
  • Monitoring client wellbeing, responding to changing health needs and supporting positive health outcomes. 
  • Working collaboratively with clients, families and multidisciplinary teams to coordinate quality care and support transitions of care. 
  • Maintaining accurate clinical documentation and ensuring compliance with professional and organisational standards 
  • Providing clinical leadership, mentoring, training and competency support to care teams and support workers. 
  • Contributing to policy development, quality improvement, risk management and accreditation activities. 

To be successful in this role you will have: 

  • Current registration as a Registered Nurse with AHPRA 
  • Experience providing nursing services within community, aged care and/or disability settings 
  • Strong clinical assessment and care planning capabilities 
  • Demonstrated experience in nursing practices relating to medication management, wound management, continence assessments, catheter care, PEG feeding and enteral nutrition 
  • Experience developing and delivering staff training and education programs 
  • Ability to assess staff competency and provide clinical supervision 
  • Experience developing policies, procedures and clinical resources 
  • Excellent communication, relationship-building and documentation skills 
  • Strong computer literacy and clinical record management skills 
  • Ability to work autonomously and as part of a multidisciplinary team 
  • Strong problem-solving and critical-thinking skills 
  • Commitment to person-centred care, safety and continuous improvement
