360 Health Management is a regional disability support provider based in Shepparton, with offices in Seymour, Bendigo, and Swan Hill, supporting wider regional areas like Mansfield and Jamieson. We’re looking for experienced Disability Support Workers to join our team supporting clients in Mansfield and surrounding areas, looking to take advantage of our diverse shift opportunities, mornings, days, evenings and weekends.

Responsibilities (depending on individual client needs)

  • Assist with daily activities such as cleaning, preparing for appointments, meal preparation, bathing, dressing, community engagement etc
  • Provide support and companionship through activities and day to day living
  • Help with mobility and transportation needs
  • Administer medication and monitor health conditions
  • Support in implement individualised care plans tailored to the client.
  • Collaborate with healthcare professionals and family members.
  • Daily case noting for shifts attended.

Essentials

  • Unrestricted Australian Drivers Licence and reliable vehicle with comprehensive insurance
  • Certification in Disability, Individual Support or another related field, Or:
  • Previous proven experience working with individuals with disabilities
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Knowledge of disability rights and advocacy
