About the Role
The Elder Care Coordinator plays a vital role in supporting Elders to navigate Aged Care pathways and access culturally safe services. This position provides trauma informed, person centred support to ensure Elders and their families understand their Aged Care Service entitlements, assessment processes, service options, and care pathways.
Working as part of the Aged & Community Care Team, the Elder Care Coordinator advocates for Elders throughout the assessment and care planning process, collaborates with Aged Care Assessors and Service Providers, and ensures Clients and Consumers receive respectful, culturally appropriate support that meets their care and safety needs.
Key responsibilities include (but not limited to):
- Providing pre assessment support and helping clients understand aged care pathways.
- Assisting Elders with My Aged Care registration and explaining entitlements and service options.
- Supporting Elders and families to prepare for assessments, plan reviews, and reassessments.
- Collaborating with Assessors and Service providers to develop and implement culturally appropriate Care Plans.
- Advocating for Elders to ensure cultural, safety, and wellbeing needs are met.
- Maintaining accurate documentation and supporting reporting requirements.
- Promote the Elder Care Support Program
This role requires someone who is proactive, culturally sensitive and respectful, and deeply aligned with Yadu Health’s mission to support First Nations health outcomes.
About You
To thrive in this position, we are looking for candidates who can meet the following criteria:
- Experience working with older Aboriginal and Torres Strait Islander people.
- Strong understanding of person centred care and trauma informed practice.
- Ability to support clients to navigate services and reduce system complexity.
- Sound understanding of My Aged Care processes and operational guidelines.
- Excellent communication and interpersonal skills, with the ability to build trust with community.
- Strong record keeping, data collection, and reporting capability.
- Ability to work autonomously with initiative and problem solving skills.
- Experience working collaboratively in a transparent, client centred team environment.
- Valid Driver Licence (Class C or equivalent).
- Have permanent work rights in Australia without needing employer sponsorship.
