About Yadu Health

Yadu Health Aboriginal Corporation is a not-for-profit, community-controlled health service providing primary health care to Aboriginal communities in Ceduna, Koonibba, and surrounding areas. About the Role

The Elder Care Connector has an important role in supporting Elders, older Aboriginal and Torres Strait Islander people, and their families to understand aged care options and get the help they need. You’ll work closely with the Aged Care Support Coordinator to connect clients with services, identify local needs, and support our community to access quality care.

Key responsibilities include:

  • Talking with community members about aged care services and outreach programs to understand and identify their needs.
  • Helping Elders and families register with My Aged Care.
  • Organising yarning circles and community meetings to share information.
  • Referring clients to the right services and supporting them through the process.
  • Working with the aged care team to improve service delivery.
  • This role requires someone who is proactive, culturally responsive, and deeply aligned with our mission to support First Nations health outcomes.

About You

We are looking for people who:

  • Have strong relationships with local Aboriginal and Torres Strait Islander communities.
  • Understand or be willing to learn about aged care and community support.
  • Understanding of person-centred care and trauma-informed practice.
  • Have experience working with elderly people.
  • Communicate well and build trust easily.
  • Be able to work both independently and as part of a team.
  • Hold a valid Driver Licence (Class C or equivalent).
