Wyanga Aboriginal Aged Care is an Aboriginal Community Controlled Organisation supporting our Elders to live independently, safely and with dignity on Country.

About the Role

This role sits at the heart of how we support our Elders to remain independent, safe and connected at home.

As a Care Partner, you’ll work closely with clients to understand their needs, goals and what matters to them, and translate that into clear, practical care plans that support independence and wellbeing.

The role focuses on care planning and case management – actively monitoring how clients are going, identifying changes early, and making sure supports remain appropriate, effective and aligned to their goals and available funding.

You’ll build trusted relationships with clients and their families, providing guidance and continuity, while working alongside the broader team to ensure services are delivered in a way that makes sense for each person.

It’s a role suited to someone who brings strong judgement, a person-centred approach, and the ability to work independently while staying connected to a team environment.

About You

You’ll bring experience in aged care, community services or a similar space, and feel comfortable working in a way that is respectful, practical and grounded.

You are someone who:

  • builds strong, trusting relationships
  • uses sound judgement and responds well to change
  • can manage a caseload and competing priorities
  • communicates clearly with clients, families and teams
  • understands the importance of culturally safe practice

Experience in care planning, case management or service coordination will be highly regarded.

Qualifications & Requirements

  • Certificate IV or higher in Aged Care, Community Services or related field
  • Current National Police Check
  • Current First Aid & CPR (or willingness to obtain)
  • Current Driver’s Licence
  • Working knowledge of aged care programs (CHSP, Home Care Packages or Support at Home preferred)
