Uplyft are seeking a passionate and experienced Social Worker / Case Worker to support grandcarers raising their grandchildren, particularly those navigating complex needs.

  • Fixed Term full-time position until 30 June 2028.

About the Role

This role delivers trauma-informed, culturally responsive case management to grandcarers raising their grandchildren, particularly those new to the role or experiencing complex needs. The focus is on supporting the safety, wellbeing, and development of children and strengthening the capacity of grandcarer families.

Working within the Uplyft Grandcare Program, you will assess needs, coordinate services, and provide tailored support, advocacy, and parenting guidance. You will collaborate with families, communities, and stakeholders to deliver integrated, person-centred support and contribute to continuous improvement and program outcomes.

The role requires a strong commitment to culturally safe practice, including respectful engagement with Aboriginal and Torres Strait Islander families, and the ability to adapt service delivery across metropolitan and regional settings.
