About the Role
This role delivers trauma-informed, culturally responsive case management to grandcarers raising their grandchildren, particularly those new to the role or experiencing complex needs. The focus is on supporting the safety, wellbeing, and development of children and strengthening the capacity of grandcarer families.
Working within the Uplyft Grandcare Program, you will assess needs, coordinate services, and provide tailored support, advocacy, and parenting guidance. You will collaborate with families, communities, and stakeholders to deliver integrated, person-centred support and contribute to continuous improvement and program outcomes.
The role requires a strong commitment to culturally safe practice, including respectful engagement with Aboriginal and Torres Strait Islander families, and the ability to adapt service delivery across metropolitan and regional settings.
What You’ll Bring
- A degree in Social Work (or recognised equivalent) and eligibility for AASW membership
- Genuine commitment to supporting children, young people and families
- Strong relationship-building skills with a strengths-based, respectful approach
- Commitment to culturally safe practice, including working with Aboriginal families and communities
- Ability to work collaboratively with families, colleagues and community partners
- Sound judgement and the ability to respond effectively to complex situations
- Well-developed communication skills and confidence engaging with diverse communities
- A reflective, learning-oriented approach with a focus on continuous improvement
