Uplyft are looking for a compassionate and capable Family Support Worker to join our team, supporting grandparents raising their grandchildren through the Grandcare Program.

About the Role

The Family Support Worker plays a key role in delivering the Grandcare Program, supporting grandparents who are raising their grandchildren in informal care arrangements. The role focuses on providing emotional, practical and financial support, including facilitating access to the Grandcarers Support Scheme (GSS) and connecting families with a range of wraparound services.

As a primary point of contact, you will engage with grandcarers who may be experiencing distress, trauma or complex family circumstances. Using a trauma-informed and culturally responsive approach, you will assess needs, guide families through eligibility processes, and link them to appropriate supports to strengthen wellbeing and stability.

About You

  • Relevant qualification or equivalent experience in community services, counselling or family support
  • Experience working with families, carers or vulnerable communities
  • Understanding of trauma, grief, financial stress and complex family dynamics affecting grandcarers
  • Strong skills in assessment, practical support, referrals and advocacy
  • Ability to manage administration, maintain accurate records and use client systems
  • Confident communicator with a warm, culturally responsive and non-judgemental approach
  • Experience or interest in group facilitation, community activities or outreach work
  • Well-organised, adaptable and able to manage competing priorities independently Commitment to trauma-informed, inclusive practice
