We’re looking for a Service Coordinator who’s passionate about people, leadership, and creating meaningful outcomes. You’ll be part of The Bridge Connects, working closely with the Connects Operations Manager and a collaborative team of leaders who value continuous learning, shared ideas, and doing things better together.
In this role, you’ll lead a supportive team to deliver person-centred, strengths-based supports across multiple locations. You’ll help create environments where participants feel understood, respected, and empowered to work towards their goals in ways that suit them.
With 3 days at our Springvale site and 2 at our Dandenong site, this role will focus on:
- Leading and supporting your team to deliver high-quality, person-led supports that reflect each participant’s goals, strengths, and preferences
- Working alongside participants to develop, implement, and review meaningful support plans and activities
- Coordinating staff rosters to ensure consistent, reliable support across locations
- Providing supervision, coaching, and feedback to help your team feel confident, supported, and able to grow
- Building positive, respectful relationships with participants, families, carers, and community connections
- Responding thoughtfully to feedback, incidents, and concerns in line with policies and a person-centred approach
- Maintaining clear, accurate records to support quality service delivery and compliance
- Supporting your team to meet NDIS Practice Standards, organisational requirements, and safe work practices
- Monitoring budgets and resources to keep services effective, sustainable, and running smoothly
- Collaborating with other leaders to share ideas, strengthen services, and contribute to continuous improvement
We are looking for a Certificate IV in disability (or higher/equivalent), a strong understanding of the various components of complex care and experience in leadership/management.
