About Us:

The Bridge is a progressive not-for-profit organisation delivering services to people with disability, and from disadvantaged backgrounds, throughout the south-eastern and northern Melbourne community. About You:

You are a compassionate and reliable professional who is genuinely committed to supporting people to live meaningful, self-directed lives. You bring a respectful, person-centred approach to your work and understand the importance of dignity, choice, and independence. You are adaptable, calm under pressure, and confident working across a variety of environments and support needs.

Key Responsibilities:

  • Support participants to access the community, build social connections, and work toward personal goals
  • Implement positive behaviour support strategies that promote independence and wellbeing
  • Collaborate with Allied Health and support networks to deliver person-centred care
  • Uphold participant choice, dignity, and duty of care
  • Assist with daily living tasks including personal care, meals, mobility and transfers
  • Communicate effectively with participants, families, and stakeholders
  • Maintain accurate documentation and record participant progress
  • Monitor and escalate changes in participant wellbeing
  • Report incidents and safety matters in line with WHS requirements
  • Update participant records, support plans, and risk assessments
  • Work in accordance with rosters and NDIS plans

Essential Selection Criteria:

  • Certificate IV in Disability or a transferable skills related course
  • Current First Aid & CPR certification
  • Ability to support wellbeing, independence, and goal achievement
  • Flexible and adaptable to changing workplace conditions
  • Strong problem-solving, creative thinking, and safety awareness
  • Ability to remain calm and seek support when needed
  • Excellent verbal and written communication skills
  • Experienced with personal care, behaviors and high intensity
  • PEG trained (desirable)
  • Capable of working independently and collaboratively
  • Proficient in MS Office, Client Management Systems, and related applications
  • Current and valid Victorian driver’s license
