The South West Aboriginal Medical Service Ltd (SWAMS) is an Aboriginal Community Controlled Health Organisation, founded on the principles of self-determination, empowerment and freedom of choice. Our mission is to provide high-quality, culturally appropriate, and holistic healthcare that builds strong and healthy Aboriginal communities. We act with integrity and are dedicated to excellence. We have over 250 staff here at SWAMS and are continuing to grow. We are actively working to “Close the Gap”.
We operate out of the Greater Bunbury area, Mandurah, and have numerous clinics throughout the South West.
SWAMS is dedicated to enhancing employment opportunities and outcomes for Aboriginal and Torres Strait Islander people. This position is designated under Section 50D of the Equal Opportunity Act 1984 (WA), and as such, only Aboriginal people are eligible to apply.
About the Role:
As an Indigenous Outreach Worker, you will play a crucial role in improving access to health services for Aboriginal and Torres Strait Islander people. Your responsibilities will include:
- Provide support and care coordination activities within an allocated case load and as directed.
- To be the primary contact point for clients being referred to other medical services from SWAMS.
- To support clients to attend their appointments.
- Work within a multidisciplinary team to provide a vital link between clients and service providers.
- Provide interagency support as required through activities such as joint case conferences.
- Liaise with other professionals and agencies in relation to the care coordination activities for clients.
- Advocate on behalf of clients on referrals and when accessing other services.
- Work within a multidisciplinary team to provide a vital link between client and service provider.
- Communicate with clients using culturally appropriate language and actions.
- Provide interagency support as required through activities such as joint case conferences.
- Document all activities related to the coordination of client care as per requirements of the organisation.
- Collect all relevant data required by the organisation.
- Work alongside other agencies and community groups to provide improved access to services to Aboriginal clients living in the community
Essential Criteria
- Aboriginal or Torres Strait Islander descent under section 50(d) Equal Opportunity Act
- Demonstrated experience and knowledge of health issues impacting on Aboriginal people.
- Demonstrated ability to communicate effectively and sensitively with Aboriginal people.
- Well-developed communication skills
- Computer skills including competence in the Microsoft office.
- Demonstrated ability to work as a team member with minimal supervision.
- Current “C” class driver’s licence.
- Relevant qualifications or equivalent in Aboriginal health work.
- Must reside in the South West and have local knowledge
about the community.
- Ability to take direction, determine priorities and manage workloads in order to meet agreed timelines and objectives.
- Demonstrated experience assisting in the development of community projects.
- Good interpersonal, written and oral communication skills.
- Demonstrated ability to work as a team member and independently with minimal supervision.
- Ability to drive to outreach to the Greater Bunbury and South West regions.
Desirable Criteria
- Current knowledge of legislative obligations for Equal Opportunity, Disability Services, Occupational Safety and Health and how these impact on employment and service delivery.
- Demonstrated front desk or medical reception experience.
- Knowledge and expertise in the use of Communicare or similar clinical database system.
