MAIN DUTIES/RESPONSIBILITIES:

• Support participant to work towards achieving their individual goals.

• Support in day to day living, cooking, housekeeping, hygiene, Medications, community access etc.

• Support participant to build their confidence.

• Liaise and collaborate with clients, their community and community support agencies.

• Monitor and review participant’s progress through the service and against the support plan.

• Utilise positive behaviour strategies

• Maintenance of progress notes and record keeping

• Support clients to participate in age-appropriate activities and tasks including employment, in home

support and community access.

• Work in accordance with the medication administration policy, work health and safety procedures and

client right’s policies including privacy and confidentiality.

• Monitor the safety and well-being of the clients and report any concerns to the Manager.

• Ensure completion of appropriate documentation. Relevant information on support provided is to be

recorded in client notes and communication book as required.

• Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.

• Work with clients, family/ carers, relevant case managers and other professionals to support the client’s

individual goals.

• In conjunction with other members of staff assist in developing, implementing and evaluating training and

behaviour programs to attain identified individual goals.

• Ensure that all programming undertaken is appropriately maintained, documented and reviewed.

• Assume the role of key worker for designated clients, including the preparation of a monthly report on

client progress.

• Liaise extensively with other staff to ensure the overall smooth and consistent running of the service.

• Report regularly to manager

• Actively contribute to a team environment through open communication, participation in regular staff

meetings, planning processes.

• Provide back up and support to other staff, as necessary.

• Notify the service if you are unable to work your rostered shifts, with as much notice as possible.

• Take reasonable care that his/her acts or omissions do not adversely affect the health and safety of other

persons.

• Report any quality and safety incident or hazards at work to their manager or Supervisor.

• Carry out roles and responsibilities as detailed in the relevant policies and processes.

3. QUALIFICATIONS AND EXPERIENCE:

• Cert III Individual Support, Mental Health, Aged Care, Community Services, Disability Work or similar.

• Minimum 6 months experience

4. COMPETENCIES:

• First aid/CPR

• Medication cert

• Manual handling cert

• Working with children check

• 100 points of ID

• NDIS clearance check

• NDIS orientation module cert

• Covid-19 Vaccination evidence

• Hand hygiene cert

• Covid-19 infection control cert
