About the Opportunity
The Elder Care Support Connector directly reports to the Practice Manager- Newman and is accountable to the Director of Clinical Services and is responsible for connecting with local Elders, older Aboriginal and Torres Strait Islander people and their families to raise awareness of their aged care entitlements. The overarching function of the ECSC is to ensure that support for clients is organized, sequenced and supported to enable effective access to appropriate aged care services both internal and external to PAMS. The ECSC will work closely with the ECS Coordinator. They will also help determine the level of local demand for aged care services, identify existing aged care services, any service gaps in the area and continue advocacy work with clients as required.

To be considered for this position, candidates must be able to clearly demonstrate their ability to meet the below Essential Selection Criteria:
SELECTION CRITERIA
ESSENTIAL:
- The person identifies as Aboriginal and/or Torres Strait Islander and is acknowledged as such by their community.
Demonstrated ability to communicate effectively and sensitively with older Aboriginal and Torres Strait Islander peoples
- Excellent and strong relationships with the local Aboriginal and Torres Strait Islander Communities
- Good communication and interpersonal skills, with the ability to build trusting relationships in community.
- Demonstrated understanding of the principles of person-cantered care and trauma-informed practice.
- Experience working with older people.
- Understanding of the internal ACCHO services available to clients.
- Knowledge of or willingness to learn about the aged care landscape.
- Ability to or willingness to learn how to enter data and reporting.
- Demonstrated experience working unsupervised and as part of a team.
- Prior use of patient information systems
In addition to the above, the desirable criteria are:
- A qualification in community services and Aboriginal mental health first aid is desirable.
The position involves working with a multicultural organisation where the majority of staff, clients and stakeholders identify as Aboriginal or Torres Strait Islander. Candidates must be able to demonstrate an understanding of the issues affecting Aboriginal and Torres Strait Islander people and their ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people.
Please note: In addition to the above skills and experience, the successful candidate will require an unrestricted WA “C” Class Driver’s Licence, and be required to submit a National Police Clearance (dated within 3 months) and Working With Children Check (where applicable) prior to appointment, in order to be eligible for the position.
Applicants for this position must be legally entitled to work in Australia and will be required to provide evidence of this entitlement. Acceptable evidence includes an Australian or New Zealand passport, birth certificate, or foreign passport with relevant visa.

This position offers the opportunity to work closely with remote Aboriginal communities and make a drastic impact to the health and well-being outcomes of these communities.
PAMS operates five sites, supported by a range of clinicians, community health workers and other ancillary workers. Although the successful candidates will have a substantive location (Newman) they must be prepared from time to time to work across any one of these sites should they be required.

The Puntukurnu Aboriginal Medical Service (PAMS) is an Aboriginal Community Controlled Health Service located in the East Pilbara region of Western Australia and provides comprehensive and culturally appropriate primary health care services to the Martu people of Newman and the four remote Aboriginal communities of Jigalong, Parnngurr, Punmu and Kunawarritji.
