Position Summary:
The NDIS Team Leader – SIL & Community Access is responsible for supporting the day-to-day delivery of quality disability support services across supported living and community-based programs. Working under the guidance of the NDIS Coordinator, the Team Leader ensures participants receive consistent, person-centred support and that staff are effectively rostered, supervised, and supported in their roles.
Key Responsibilities:
Service Delivery & Support Coordination
-Oversee the implementation of individual support plans across SIL and community access services.
-Ensure participant routines, goals, and preferences are upheld in daily practice.
-Escalate concerns or changes in participant needs to the NDIS Coordinator promptly.
Team Leadership & Supervision
-Lead, mentor, and provide on-the-ground support to support workers across multiple service settings.
-Conduct regular supervision, check-ins, and debriefs with team members.
Promote a culture of accountability, respect, and professional development.
Rostering & Shift Management
-Create and maintain efficient rosters that align with participant funding and support ratios.
-Ensure adequate shift coverage for planned and unplanned leave across all SIL and community access locations.
-Monitor shift attendance and manage on-call or after-hours support needs where relevant.
Compliance & Reporting
-Ensure services comply with the NDIS Practice Standards, participant service agreements, and internal policies.
-Complete and review incident reports, medication documentation, and participant progress notes.
-Support audit preparation and contribute to continuous improvement initiatives.
Communication & Collaboration
-Act as the first point of contact for frontline staff regarding daily operational issues.
-Liaise regularly with the NDIS Coordinator to report on service delivery outcomes and staffing matters.
-Foster positive relationships with participants, families, guardians, and allied health providers.
Selection Criteria:
Essential:
-Minimum 2 years’ experience in disability services, with leadership or supervisory responsibilities.
-Experience in SIL or community access programs under the NDIS.
-Strong rostering and scheduling capabilities (ShiftCare, Skedulo, etc.).
-Well-developed understanding of person-centred active support and NDIS compliance.
-Excellent written, verbal, and interpersonal communication skills.
Desirable:
-Certificate IV or higher in Disability, Community Services, or related field.
-Current NDIS Worker Screening Check, Working with Children Check, First Aid & CPR.
-Driver’s licence and access to a reliable vehicle.
