Summary:

We are seeking a compassionate and experienced Palliative Care Nurse to join our team. The successful candidate will provide specialised nursing care to patients with life-limiting conditions, focusing on enhancing their quality of life through pain and symptom management, emotional support, and holistic care. The Palliative Care Nurse will work closely with patients, families, and interdisciplinary teams to deliver comprehensive and empathetic care.

Responsibilities:

  • Deliver specialised nursing care to patients with terminal or life-limiting illnesses, focusing on symptom management, pain relief, and comfort.
  • Conduct comprehensive assessments of patients’ physical, emotional, and psychological needs, and develop individualised care plans.
  • Administer medications, manage complex symptoms, and provide treatments in accordance with care plans and medical directives.
  • Provide emotional and psychological support to patients and their families, addressing concerns, fears, and coping strategies.
  • Collaborate with a multidisciplinary team, including doctors, social workers, and chaplains, to coordinate and integrate palliative care services.
  • Educate patients and families on disease progression, care options, and self-care techniques, empowering them to make informed decisions.
  • Monitor and document patients’ progress, responses to treatments, and changes in condition, ensuring accurate and timely records.
  • Ensure compliance with relevant legislation, regulations, and organisational policies related to palliative care.
  • Advocate for patients’ preferences and dignity in end-of-life care, ensuring that their wishes are respected and incorporated into care plans.
  • Participate in quality improvement initiatives and contribute to the development of best practices in palliative care.

Qualifications:

  • Bachelor’s degree in Nursing or equivalent; postgraduate qualifications in palliative care or a related field preferred.
  • Registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.
  • Proven experience in palliative care or a related specialty, with a strong understanding of end-of-life care principles.
  • Excellent clinical assessment and problem-solving skills, with the ability to manage complex symptoms and care needs.
  • Compassionate and empathetic approach to patient care, with strong communication and interpersonal skills.
  • Ability to work effectively both independently and as part of a multidisciplinary team.
  • Proficiency in using electronic health records and other relevant documentation tools.
