Summary:

We are seeking a highly organised and compassionate Home Care Coordinator to join our team. The successful candidate will be responsible for managing and overseeing the delivery of home care services to clients, ensuring they receive personalised and high-quality care. The Home Care Coordinator will work closely with clients, their families, and care staff to ensure effective service delivery and meet individual care needs.

Responsibilities:

  • Coordinate and manage the delivery of home care services, including personal care, nursing, and support services, tailored to individual client needs.
  • Develop and implement personalised care plans in collaboration with clients, families, and healthcare professionals.
  • Schedule and supervise care staff, ensuring that care is delivered according to the care plan and in compliance with relevant regulations and standards.
  • Monitor and review client progress, adjusting care plans as needed to address changes in health status or care requirements.
  • Conduct regular assessments of clients’ needs and preferences, maintaining accurate and up-to-date records.
  • Provide support and guidance to care staff, including training and performance management.
  • Liaise with clients, families, and healthcare providers to address any concerns or issues related to care services.
  • Ensure compliance with all relevant legislation, regulations, and organisational policies related to home care services.
  • Prepare and maintain documentation, including care plans, incident reports, and care logs.
  • Promote a positive and respectful environment for clients, ensuring their dignity and independence are upheld.

Qualifications:

  • Bachelor’s degree in Nursing, Social Work, Allied Health, or a related field.
  • Relevant experience in home care or community services, with a strong understanding of care coordination and management.
  • Excellent organisational and time management skills, with the ability to handle multiple tasks and priorities.
  • Strong communication and interpersonal skills, with the ability to build relationships with clients, families, and care staff.
  • Proficiency in using care management software and other relevant tools.
  • Knowledge of relevant legislation, regulations, and best practices in home care services.
  • Compassionate, empathetic, and client-focused approach to care.
