The Role

We are seeking an experienced Senior Case Manager who is responsible to ensure timely and efficient allocation of support workers to participants receiving Support at Home. The Senior Case Manager collaborates closely with the care management team, support workers, and participants to ensure the delivery of high-quality care services that meet individual needs and preferences.

Key Responsibilities

  • Review and provide specialist input on client care plans, ensuring alignment with individual needs, goals, and evidence-based best practice standards.
  • Advise care teams on complex care situations, including changes in client physical, cognitive, or mental health status.
  • Identify and advise on solutions to complex scheduling challenges, including conflicts, gaps in coverage, or high-risk client scenarios.
  • Coordinate and manage the scheduling of client visits and support worker assignments, taking into account factors such as availability, skills, geographic location, and client preferences.
  • Develop, allocate and manage financial resources and budget utilisation and monitor surplus/deficit strategies.

About You

  • Experience in Support at Home or relevant sectors
  • Knowledge of the NDIS and/or Aged Care sector (especially self-managed NDIS & HCP).
  • Excellent communication and time management skills.
  • Confident computer skills (rostering systems or CRMs a bonus).
  • Police Check, WWCC, NDIS Worker Screening Check (or willingness to obtain).
  • Current First Aid & CPR (or willingness to obtain).
  • Minimum of two doses of COVID-19 vaccination.
  • Comfortable in fast-paced, regulated, or changing environments
  • Proactive and committed to continuous improvement
