About the Role:
You will support participants across a variety of settings, helping them live with independence, dignity and choice.
You may work with individuals with:
• Physical disabilities such as cerebral palsy
• Neurological conditions such as epilepsy including seizure support
• Cognitive conditions such as dementia
• Mental health conditions such as schizophrenia, anxiety, and bipolar disorder
• Complex behaviours requiring Positive Behaviour Support Plans
• Mealtime management including safe food preparation and mealtime support
• Medication management in line with care plans and compliance requirements
Your role:
• Provide, consistent person centred support while maintaining professionalism
• Structured and strong communication
• Assisting with personal care daily living tasks
Service Areas you may be rostered across:
Supported Independent Living (SIL)
- 24/7 structured environments
- Daily routines skill building and independence
- Team based support with clear handovers
In Home and Self Care Support
- Personal care such as showering toileting and grooming
- Daily living assistance in a participant’s home
- Supporting routines, health and wellbeing
Community Access
- Appointments, social activities and outings
- Building confidence and independence
- Goal focused participation
Complex and Behaviour Support
- Supporting participants with behaviours of concern
- Implementing structured PBSP strategies
- Maintaining calm, consistent, and proactive approaches
Availability and Flexibility to succeed in this role you must be:
✔️ Flexible across days times and service types
✔️ Open to on call shifts when required
✔️ Willing to travel 45 minutes to 1 hour between participants (not mandatory)
✔️ Committed to roster stability and reliability
We prioritise Support Workers who can build consistent ongoing relationships with participants.
Key Responsibilities:
- Deliver high-quality person-centered support
- Implement Positive Behaviour Support Plans
- Provide personal care and daily living assistance
- Support medication administration and health monitoring
- Respond appropriately to seizures and health related needs
- Assist with community access and participant goals
- Complete accurate documentation including progress notes and incident reports
- Follow compliance, WHS and organisational procedures
- Work collaboratively with team leaders and allied health professionals
What We Are Looking For:
- Experience in disability, community, SIL or complex care support
- Understanding of person centered and trauma informed practice
- Strong communication and documentation skills
- Calm, professional, and reliable approach
- A genuine commitment to consistency and quality support
Mandatory Requirements:
- Yellow Card & Blue Card
- NDIS Worker Orientation Module
- NDIS module Supporting Safe and Enjoyable Meals
- First Aid and CPR
- Medication Competency
- Registered Vehicle with Comprehensive Insurance
- Valid driver’s license
Desirable Qualifications:
- Certificate III or IV in Individual Support or equivalent in any of the following: Aged Care, Mental Health Support, Nursing, Psychology, Community Services
