Job Overview:
We are looking for a passionate and dedicated NDIS Support Coordinator and Psychosocial Recovery Coach to join our team. In this role, you will assist NDIS participants with varying abilities in achieving their NDIS plan goals, enhancing their daily living skills, and promoting their overall well-being. You will report directly to the Capacity Building Team Lead, who is committed to supporting your professional growth and success.

The role is based at the Jandakot office, with potential for an alternative location at Shenton Park, and includes community visits across multiple locations.

Key Responsibilities of this role include but not limited to
NDIS Support Coordination:
- Assist NDIS participants in understanding and implementing their NDIS plans
- Collaborate with participants to identify goals and supports needed to achieve them.
- Liaise with service providers and coordinate the delivery of NDIS-funded services.
- Maintain accurate records and documentation of participant progress and services provided.
Psychosocial Recovery Coaching:
- Provide one-on-one psychosocial recovery coaching to help participants build resilience and improve mental health.
- Develop recovery plans and strategies tailored to each participant’s unique needs
- Offer emotional support, and skill-building to enhance coping and self-management.
- Monitor and evaluate the effectiveness of coaching interventions and adjust plans as needed.
To be successful in this position you will have –
- 1 to 3 years’ demonstrated experience in direct case management in the NDIS sector
- Background in Social Work, Psychology, Occupational Therapy, Community Services, Human Services, Counselling, Allied Health or similar fields are highly regarded.
- Strong understanding of NDIS Quali ty and Safeguards Commission requirements, including documentation, standards, timelines, and audit readiness
- Ability to work autonomously within a structured clinical governance framework, including supervision, peer review, and continuous improvement processes.

- Valid NDIS Worker Screening Check
- Unrestricted work rights in Australia
- Valid WA Driver’s license and Licensed and Insured vehicle
- Current and valid First Aid & CPR certificates
- Current and valid Working With Children Check
