Job Overview
The Home Coordinator provides direct, person-centered support to clients in Supported Independent Living (SIL) settings, assists with essential administrative and compliance tasks, and supports the effective functioning of the home. This role works under the guidance of the Regional Manager SIL and contributes to a positive, safe, and inclusive environment for clients and staff.
Acting as a trusted representative of Mosaic, the SIL Home Coordinator builds strong relationships with clients, families, staff, and the broader community, consistently upholding Mosaic’s reputation and purpose.

Key Responsibilities of this role include but not limited to
- Deliver direct, person-centred support to clients.
- Implement rosters and ensure shift coverage across all allocated homes.
- Support with onboarding new clients, develop and accurately maintain client documentation including the relevant NDIS documentation.
- Conduct basic audits, maintain onsite checklists
- Onboard new support workers that include effective induction in line with Mosaic’s policies and procedures, and client needs.
- Mentor and coach the support workers that includes ongoing support and supervision leading towards their performance development.
- Manage and approve leave in line with Mosaic leave procedures.
- Address performance issues, grievances, and safety incidents in a timely manner in partnership with People & Culture.
- Provide after-hours on-call support on a rostered basis.
- Conduct regular meetings, supervisions and annual performance reviews.
- Perform daily responsibilities in line with Mosaic’s values and promote cultural safety, inclusion and respectful practice.
- Perform effectively in compliance requirements including staff adherence to information and documentation standards, contribute to continuous improvement, and assist with internal and external reporting.
To be successful in this position you will have –
- Diploma in Disability, Community Services, or a related discipline (essential); or a relevant tertiary degree is highly desirable.
- Minimum 3 years’ experience in the disability, aged care, or community services sector.
- Proven ability to lead, coach, and develop diverse teams to deliver high-quality, person-centred support.
- Experience supporting clients with complex needs, including behaviours of concern.
- Proficient knowledge in Microsoft Office and client management systems.
