Location: Dandenong, Casey, Cardinia LGAs – based in Narre Warren

Time Fraction: Full-time

Contract: Fixed-term until 30 June 2029

About the Care Finder Program:

We are seeking a Care Finder Coordinators. The Care Finder Program supports older Australians who face barriers to accessing aged care services. Our Care Finder team provides proactive outreach, assists with navigating My Aged Care processes, and connects people to appropriate health and community supports. We work with individuals who may be socially isolated, experiencing homelessness, or otherwise vulnerable.

This role is suited to someone who demonstrates strong empathy and a calm, professional approach when supporting older people in complex or stressful situations. You will be confident engaging with service providers and advocating for the needs of clients.

Experience working with older people from culturally and linguistically diverse backgrounds is highly regarded, including the ability to work effectively with interpreters or communicate in a community language.

  • About the Roles:

Care Finder Coordinator:

  • Deliver person-centred support to older Australians facing barriers to aged care access
  • Assist clients with My Aged Care registration, assessment, and service connection
  • Build referral pathways with health and community services
  • Advocate for clients and promote ongoing engagement with services

About You:

  • Relevant qualification in Social Work, Community Services, Aged Care, or related field (Advanced Diploma minimum)
  • Experience in client assessment, care planning, and coordination
  • Knowledge of My Aged Care processes and aged care service system
  • Strong interpersonal and communication skills
  • Cultural awareness and sensitivity
  • Additional Requirements:
  • Current Victorian Driver’s License and insured vehicle
  • National Police Check and Working with Children Check (or willingness to obtain)
  • Right to work in Australia
  • First Aid/CPR
