The Role
The Youth & Family Case Management Service provides intensive, outreach-based support to children, young people and their families across the ACT. The service focuses on assisting families experiencing complex challenges such as homelessness, unemployment, substance use, domestic and family violence, and trauma.
Support is provided to children and young people involved with the child protection system, those transitioning from out of home care, Aboriginal and Torres Strait Islander families, and people from culturally and linguistically diverse backgrounds.
The service is child and young person centred, family focused, and works collaboratively with community organisations, government agencies and schools to provide coordinated, holistic support.
This role requires the ability to work independently and as part of a small team, with strong skills in case management, service coordination, engagement, advocacy, referrals, administration and time management.
About You
We are seeking a motivated and compassionate professional who is committed to supporting vulnerable children, young people and families experiencing complex challenges.
To be successful in this role, you will have
- Relevant tertiary qualifications with at least 2 years’ experience supporting children, young people and families with complex needs.
- Understanding of the key issues impacting vulnerable and disadvantaged children, young people and their families.
- Knowledge of collaborative practice and service coordination within a multi-agency environment.
- Strong written and verbal communication, administration and IT skills.
- Understanding of relevant legislation, standards and frameworks including EEO, Work Health & Safety, privacy and confidentiality.
- Ability to work independently and within a small team while managing competing priorities.
- Demonstrated resilience, empathy, emotional intelligence and commitment to ethical practice and social equity.
- Reliable, adaptable and able to work under pressure while meeting deadlines.
Current Driver Licence and a valid Working With Vulnerable People registration.
