About the Role
We’re looking for an experienced and compassionate Registered Nurse to join us, working across home and community settings to support our growth in Southeast region.
This is a dynamic, field-based role – not a desk job. You’ll be out in the community, supporting clients in their homes and local settings, delivering high-quality clinical care and thoughtful case management.
You’ll work with people living with disability, those recovering from injury, and individuals requiring age-related care, ensuring support is personalised, respectful and aligned with NDIS and Support at Home (SAH) funding.
What You’ll Be Doing
As a Clinical Care Partner, you’ll play a key role in:
- Completing initial and ongoing assessments to identify client needs and align support with funding requirements in home and community settings
- Developing and reviewing complex care plans, coordinating services with internal teams and external stakeholders
- Providing nursing and complex care support, including on-site assessment for high-care clients
- Offering clear, confident clinical guidance to staff and clients to ensure safe, high-quality care
- Monitoring client notes, incidents and service trends to maintain strong clinical standards
- Responding to client concerns, incidents and feedback with professionalism, empathy and accountability
- Coaching and training Support Workers, including competency assessments
- Collaborating with families, Support Workers and allied health professionals to support long-term wellbeing
About You
To succeed in this role, you’ll bring:
- A tertiary qualification in Nursing with current AHPRA registration
- Strong understanding of NDIS Practice Standards, aged care frameworks, person-centred planning and risk management
- Prior experience working in disability or aged care services (Preferably)
- A valid Australian Driver Licence (community travel is essential)
- Excellent communication and interpersonal skills, grounded in respect and collaboration
- Strong organisation and decision-making skills, especially in fast-paced environments
- A genuine commitment to improving the lives of people living with disability
