JewishCare is a leading, child-safe social services organisation supporting the resilience and independence of individuals and families across the NSW community.
We are seeking an experienced Clinical Intake & Assessment Officer to join our Community & Engagement team on a 6-month full-time fixed-term contract. This is a high-impact role ideal for a clinically trained professional who thrives in fast-paced intake, triage and assessment environments.
About the role
As the first point of contact for new referrals, you will conduct intake and triage assessments across mental health, aged care, disability, and child & family services. Using JewishCare’s clinical triage framework, you will assess presenting issues and risk, coordinate referrals, and ensure clients experience continuity of care and timely access to the right supports.
You will be supported with professional supervision and development while working across diverse client cohorts.
Key responsibilities
- Conduct intake, triage and brief clinical assessments
- Assess presenting issues, risks and service eligibility
- Triage clients in and out of services, providing warm referrals
- Act as an ongoing point of contact for clients during intake
- Liaise with GPs, NGOs and partner services
- Maintain accurate, timely and confidential client documentation
- Participate in intake meetings and support service navigation
- Contribute positively to service improvement and change initiatives
Essential criteria
- Bachelor or postgraduate qualification in Social Work or Psychology, and Community Welfare
- Current professional registration
- Minimum 2 years’ experience in intake and/or triage roles
- Ability to conduct brief assessments and support diagnostic formulation
- Strong communication, organisational and documentation skills
- Compassionate, client-centred and emotionally intelligent approach
- Knowledge of relevant legislation and mandatory reporting requirement
- Experience in one or more in mental health, aged care, disability or NDIS services
- Knowledge of trauma-informed and person-centred practice
