We’re looking for someone with case management experience to provide support to our participants and their supporters.

The Care Partner is responsible for accepting referrals, arranging and facilitating the delivery of in-home services and supports for people assessed as eligible to receive Commonwealth Home Support Program (CHSP) services or Fee-for-Service programs (DVA – Veterans Home Care and Home and Community Care Program for Younger People [HACC-PYP]).

This role ensures that older people and their carers are supported to maximise independence, wellbeing and participation in community life, while remaining safe and well at home. The Care Partner upholds the principles of wellness, reablement and restorative care.

This is a permanent full-time role based out of our Bendigo, Echuca, Maryborough, Swan Hill, Seymour or Gisborne office.

This position has been classified under the Social, Community, Home Care and Disability Services Award as Level 4 with a starting annual salary of $88,090 + Superannuation + Salary Packaging.
